Our Privacy Policy and Privacy Rights of Patients

In our practice we respect each patient’s right to privacy concerning his or her dental record. All information regarding your (or your child's) health, dental treatment needs, and treatment history is considered "protected health information." We do not disclose or discuss any of this information with anyone without your specific written permission. Furthermore, you are entitled to unlimited access to your or your child's dental records.
There are two types of written authorizations that you can use to permit us to discuss or disclose your protected health information.
 

to a Health Care Provider    
The first authorization gives us your consent to use and disclose any protected health information (usually to other doctors and insurance companies) for the purpose of facilitating treatment or collecting payments.  The new federal laws regulating health information privacy and access permit this type of disclosure without your specific authorization.
        

to a non-Health Care Provider
The second type of authorization permits us to release only very specific information to specific individuals (other than physicians, dentists and insurers) designated by you. Such individuals, for example, may be an employer or a child's school. With this type of authorization your consent to release or disclose protected health information must identify specific information to be released and a specific end date at which the release terminates.
